Une approche philosophique semble essentielle pour mieux saisir ces enjeux, ce que propose la philosophie de la psychiatrie.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"sect0015\">Objectifs<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Pour faciliter l\u2019initiation \u00e0 la philosophie de la psychiatrie, cet article propose une cartographie p\u00e9dagogique articul\u00e9e autour de quatre \u00ab&nbsp;mouvements&nbsp;\u00bb pour penser la psychiatrie&nbsp;: expliquer, d\u00e9crire, interpr\u00e9ter et transformer. Une telle proposition a pour objectif d\u2019offrir aux cliniciens des outils philosophiques pour situer, analyser et enrichir leur r\u00e9flexion clinique ou de recherche.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"sect0020\">M\u00e9thodes<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Les apports respectifs et compl\u00e9mentaires de quatre traditions philosophiques associ\u00e9es \u00e0 ces mouvements sont pr\u00e9sent\u00e9s&nbsp;: l\u2019\u00e9pist\u00e9mologie psychiatrique (\u00ab&nbsp;expliquer&nbsp;\u00bb), la ph\u00e9nom\u00e9nologie psychiatrique (\u00ab&nbsp;d\u00e9crire&nbsp;\u00bb), l\u2019herm\u00e9neutique psychiatrique (\u00ab&nbsp;interpr\u00e9ter&nbsp;\u00bb) et la psychiatrie critique (\u00ab&nbsp;transformer&nbsp;\u00bb).<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"sect0025\">R\u00e9sultats<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Une pr\u00e9sentation de l\u2019origine historique, des objectifs et des principaux th\u00e8mes est propos\u00e9e pour chacun de ces quatre courants, accompagn\u00e9e de r\u00e9f\u00e9rences bibliographiques cl\u00e9s. Non exclusifs, ces mouvements doivent \u00eatre consid\u00e9r\u00e9s comme une boussole p\u00e9dagogique pour orienter l\u2019exploration du champ de la philosophie de la psychiatrie, sans pr\u00e9tendre en r\u00e9duire la complexit\u00e9 ou la richesse.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"sect0030\">Discussion et conclusion<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Une telle cartographie p\u00e9dagogique peut permettre aux \u00e9tudiants et praticiens en psychiatrie d\u2019apprendre \u00e0 nuancer leurs croyances et repr\u00e9sentations, en d\u00e9veloppant une culture de l\u2019incertitude et de l\u2019humilit\u00e9, encourageant le dialogue interdisciplinaire, guidant les questionnements conceptuels et, par extension, soutenant les questions \u00e9thiques inh\u00e9rentes \u00e0 la discipline.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Abstract<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"sect0040\">Context<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Clinical psychiatry works at the intersection of biological, social, and human sciences, addressing complex conceptual, methodological, and normative issues. These include for instance the definition of psychiatric disorders, the role of values in classifications, and the power dynamics in clinical practices. A philosophical approach for psychiatry seems essential to better understand these challenges and provide tools for addressing them.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"sect0045\">Objectives<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">This article proposes a pedagogical framework (a \u201cmap\u201d) for structuring clinical psychiatry, based on four \u201cmovements\u201d derived from philosophy of psychiatry: explaining, describing, interpreting, and transforming. The goal of such a map is to offer clinicians intellectual tools to contextualize and analyse any question, reflection, or research project in psychiatry on a philosophical basis.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"sect0050\">Methods<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The complementary contributions of four major philosophical traditions are presented: psychiatric epistemology (\u201cexplaining\u201d), phenomenological psychiatry (\u201cdescribing\u201d), hermeneutic psychiatry (\u201cinterpreting\u201d), and critical psychiatry (\u201ctransforming\u201d).<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"sect0055\">Results<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">A presentation of the historical origins, objectives, and main themes is provided for each of these four movements, along with key bibliographic references. These four movements allow for the articulation of scientific knowledge, subjective patient experiences, narrative analysis, institutional critique, the exploration of normative dimensions in diagnostics, the interrogation of power dynamics in therapeutic relationships, or reflection on the interactions between medical and social norms&nbsp;\u2013&nbsp;to name a few. For example, epistemological approaches seek to explain psychiatric phenomena by dissecting their components and establishing causal relationships, such as examining the neurobiological underpinnings of specific disorders or the conceptual frameworks underlying diagnostic categories. Phenomenological approaches focus on the subjective lived experiences of patients, while hermeneutic approaches analyse patient narratives as texts shaped by social, cultural, and historical influences. Similarly, critical psychiatry challenges the structural and normative aspects of institutions, aiming to transform practices for more inclusive and equitable care.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"sect0060\">Discussion and conclusion<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Such a pedagogical mapping can help students and practitioners in psychiatry to temper their assumptions by promoting a culture of uncertainty, encouraging interdisciplinary dialogue, guiding conceptual inquiry, and, by extension, supporting the ethical questions inherent to the discipline. En raison de ces enjeux, il semble essentiel que chaque \u00e9tudiant et praticien en psychiatrie puisse engager un travail r\u00e9flexif, personnel ou collectif sur la pratique psychiatrique, prenant en compte les enjeux th\u00e9oriques qui concernent cette sp\u00e9cialit\u00e9 m\u00e9dicale et qui peuvent guider, plus ou moins explicitement, l\u2019approche clinique ou de recherche de cette discipline. Loin d\u2019un savoir th\u00e9orique d\u00e9connect\u00e9 des pratiques, cet apprentissage peut permettre d\u2019am\u00e9liorer les prises de d\u00e9cision clinique et la qualit\u00e9 des soins et de la recherche&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0410\">[1]<\/a>,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0415\">[2]<\/a>,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0555\">[30]<\/a>. De plus, au-del\u00e0 de la pratique clinique, la philosophie de la psychiatrie propose un cadre didactique et p\u00e9dagogique pour l\u2019enseignement, encourageant le dialogue interdisciplinaire, guidant les questionnements conceptuels et, par extension, soutenant les questions \u00e9thiques inh\u00e9rentes \u00e0 la discipline.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cependant, se rep\u00e9rer au sein de ce champ philosophique peut \u00eatre difficile, \u00e9tant donn\u00e9 la multiplicit\u00e9 des travaux, des publications et des perspectives offertes par les recherches sur ce sujet. L\u2019enjeu du pr\u00e9sent article est de fournir une cartographie conceptuelle et m\u00e9thodologique pour rendre accessible un tel champ aux \u00e9tudiants et praticiens en psychiatrie, sans pour autant chercher \u00e0 en r\u00e9duire la complexit\u00e9, la richesse et la profondeur. Nous souhaitons ainsi proposer une cartographie p\u00e9dagogique g\u00e9n\u00e9rale destin\u00e9e aux cliniciens d\u00e9sireux de s\u2019initier \u00e0 la philosophie de la psychiatrie.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">De fait, dans cet article, nous proposons de pr\u00e9senter quatre \u00ab&nbsp;mouvements&nbsp;\u00bb pour explorer la philosophie de la psychiatrie. Pour chacun de ces mouvements, nous rappellerons leur perspective historique, leurs objectifs et les principaux th\u00e8mes qu\u2019ils abordent, tout en proposant des r\u00e9f\u00e9rences bibliographiques cl\u00e9s. L\u2019objectif de cet article est d\u2019offrir un cadre de travail pratique, structur\u00e9, que les \u00e9tudiants et praticiens en psychiatrie pourront utiliser comme point de d\u00e9part pour contextualiser et analyser toute question, r\u00e9flexion ou travail de recherche en psychiatrie sur une base philosophique. Une telle cartographie, orient\u00e9e par des choix \u00e0 vis\u00e9e didactique pour aider le clinicien novice \u00e0 se rep\u00e9rer dans le champ, se veut davantage un panorama qu\u2019une synth\u00e8se, et est donc n\u00e9cessairement imparfaite&nbsp;\u2013&nbsp;tout en offrant des rep\u00e8res essentiels \u00e0 l\u2019exploration du domaine.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"sect0080\">2.&nbsp;Les quatre principaux mouvements en philosophie de la psychiatrie<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">La philosophie de la psychiatrie peut \u00eatre approch\u00e9e \u00e0 travers quatre grands mouvements, ou quatre grands gestes philosophiques pour la psychiatrie, qui sont issus d\u2019un tr\u00e9pied m\u00e9thodologique. La proposition de cette cartographie s\u2019appuie sur (1) six ann\u00e9es d\u2019exp\u00e9rience et de discussions p\u00e9dagogiques au sein du Dipl\u00f4me Inter-Universitaire \u00ab&nbsp;Philosophies de la psychiatrie&nbsp;\u00bb, (2) sur les r\u00e9sultats d\u2019une \u00e9tude quantitative \u00e9valuant les comp\u00e9tences philosophiques des \u00e9tudiants et praticiens en psychiatrie&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0585\">[36]<\/a>, ainsi que (3) sur la litt\u00e9rature en philosophie et sociologie des sciences, en suivant l\u2019organisation des manuels et trait\u00e9s dans ces disciplines&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0440\">[7]<\/a>,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0590\">[37]<\/a>,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0695\">[58]<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Chacun de ces mouvements de la philosophie de la psychiatrie offre une perspective diff\u00e9rente sur la pratique, la clinique et la recherche en psychiatrie&nbsp;:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>\u00ab\u00a0expliquer\u00a0\u00bb\u00a0: ce mouvement analyse la constitution du savoir en psychiatrie en tant que discipline scientifique. Il s\u2019agit de chercher \u00e0 expliquer, justifier, d\u00e9composer et mettre en relation les composantes des objets et des concepts psychiatriques\u00a0<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0790\">[77]<\/a>. Ce mouvement s\u2019ancre dans une tradition li\u00e9e \u00e0 l\u2019\u00e9pist\u00e9mologie psychiatrique\u00a0;<\/li>\n\n\n\n<li>\u00ab\u00a0d\u00e9crire\u00a0\u00bb\u00a0: ce mouvement cherche \u00e0 d\u00e9crire l\u2019exp\u00e9rience psychiatrique telle qu\u2019elle est v\u00e9cue par le patient ou le clinicien\u00a0<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0610\">[41]<\/a>. Pour ce mouvement, la pratique psychiatrique a une l\u00e9gitimit\u00e9 \u00e9pist\u00e9mique parce qu\u2019elle est construite \u00e0 partir de l\u2019exp\u00e9rience humaine. Ancr\u00e9 dans la tradition de la ph\u00e9nom\u00e9nologie psychiatrique, il cherche \u00e0 mieux cerner, comprendre et transmettre les exp\u00e9riences individuelles, intersubjectives et collectives afin de fonder la psychiatrie sur des bases anthropologiques\u00a0;<\/li>\n\n\n\n<li>\u00ab\u00a0interpr\u00e9ter\u00a0\u00bb\u00a0: ce mouvement interroge la dimension interpr\u00e9tative de la pratique psychiatrique. Il est inspir\u00e9 par des \u00e9tudes litt\u00e9raires et par l\u2019herm\u00e9neutique psychiatrique, c\u2019est-\u00e0-dire l\u2019art de l\u2019interpr\u00e9tation des r\u00e9cits et des v\u00e9cus permettant d\u2019en \u00ab\u00a0d\u00e9coder\u00a0\u00bb le sens, la symbolique ou les significations et les comprendre en tant qu\u2019exp\u00e9rience humaine\u00a0<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0465\">[12]<\/a>,\u00a0<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0730\">[65]<\/a>\u00a0;<\/li>\n\n\n\n<li>\u00ab\u00a0transformer\u00a0\u00bb\u00a0: ce mouvement con\u00e7oit la psychiatrie comme un espace \u00e0 r\u00e9inventer, en proposant une relecture critique des pratiques et des structures institutionnelles. Ancr\u00e9 dans la tradition de la \u00ab\u00a0psychiatrie critique\u00a0\u00bb\u00a0<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0520\">[23]<\/a>,\u00a0<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0700\">[59]<\/a>,\u00a0<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0795\">[78]<\/a>, d\u00e9ploy\u00e9 en pratique en France, par exemple, au sein des courants de \u00ab\u00a0psychoth\u00e9rapie institutionnelle\u00a0\u00bb\u00a0<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0725\">[64]<\/a>, il d\u00e9fend des soins qui se veulent \u00e9mancipateurs et respectueux de la dignit\u00e9 des patients et des praticiens, en \u00e9tant notamment attentif aux dynamiques de pouvoir.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Ces quatre mouvements de la philosophie de la psychiatrie s\u2019inscrivent dans l\u2019histoire de la philosophie et de la sociologie (des sciences), repr\u00e9sent\u00e9s par exemple par Carl Hempel (pour la diff\u00e9rence entre interpr\u00e9ter et expliquer), par Wilhelm Dilthey et Max Weber (qui ont propos\u00e9 et analys\u00e9 l\u2019opposition classique entre expliquer et comprendre&nbsp;\u2013&nbsp;avant d\u2019interpr\u00e9ter)&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0515\">[22]<\/a>, par d\u2019autres sociologues plus contemporains (comme Eva Illouz, qui a distingu\u00e9 l\u2019interpr\u00e9tation, l\u2019explication et la transformation dans le cadre de ses travaux sur les \u00e9motions et le capitalisme)&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0655\">[50]<\/a>, ou des th\u00e9oriciens de diverses \u00c9coles (comme les penseurs de l\u2019\u00c9cole de Francfort dans leurs travaux sur la description des structures sociales et leur transformation critique)&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0600\">[39]<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Dans la suite de cet article, nous commencerons par pr\u00e9senter en d\u00e9tail ces quatre mouvements pour interroger la psychiatrie au travers du prisme de la philosophie de la psychiatrie. Ensuite, \u00e0 l\u2019aide de quelques exemples, nous illustrerons leur compl\u00e9mentarit\u00e9 et leur intrication. L\u2019\u00e9pist\u00e9mologie a ainsi cherch\u00e9 \u00e0 clarifier les concepts, analyser la structure des discours scientifiques et s\u2019est interrog\u00e9e sur la mani\u00e8re de justifier rationnellement une id\u00e9e ou un concept&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0420\">[3]<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Historiquement tributaire du positivisme d\u2019Auguste Comte, la philosophie analytique cherchait \u00e0 privil\u00e9gier une approche \u00ab&nbsp;scientifique&nbsp;\u00bb et \u00ab&nbsp;explicative&nbsp;\u00bb du monde, fond\u00e9e sur l\u2019empirisme, l\u2019analyse logique (du langage) et la recherche de clart\u00e9 conceptuelle&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0475\">[14]<\/a>. Le cercle de Vienne, sous l\u2019impulsion de Rudolf Carnap et influenc\u00e9 par les travaux de Ludwig Wittgenstein, a ainsi d\u00e9velopp\u00e9 un programme visant \u00e0 \u00ab&nbsp;unifier les sciences&nbsp;\u00bb&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0805\">[80]<\/a>. Les limites du \u00ab&nbsp;positivisme logique&nbsp;\u00bb de ce cercle analytique, notamment sa difficult\u00e9 \u00e0 traiter des questions normatives (c\u2019est-\u00e0-dire sur ce qu\u2019il faudrait faire ou croire) et m\u00e9taphysiques (c\u2019est-\u00e0-dire sur ce qui existe r\u00e9ellement)&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0745\">[68]<\/a>, ont conduit \u00e0 un \u00e9largissement de la philosophie analytique vers d\u2019autres champs comme l\u2019\u00e9pist\u00e9mologie, la philosophie du langage ou la philosophie de l\u2019esprit.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Pour la psychiatrie, l\u2019\u00e9pist\u00e9mologie a ainsi contribu\u00e9 \u00e0 restructurer les fronti\u00e8res entre les sciences humaines et les sciences naturelles, tout en restant fid\u00e8le \u00e0 son id\u00e9al de clart\u00e9 argumentative et de pr\u00e9cision m\u00e9thodologique.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"sect0095\">3.2.&nbsp;Objectifs en psychiatrie<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Un des principaux objectifs de l\u2019\u00e9pist\u00e9mologie psychiatrique est de chercher \u00e0 expliquer les ph\u00e9nom\u00e8nes psychiatriques, et notamment les troubles psychiatriques, \u00e0 travers une d\u00e9composition et une mise en relation de leurs composantes, en utilisant par exemple des objets issus des neurosciences, de la s\u00e9miologie et de l\u2019\u00e9pist\u00e9mologie appliqu\u00e9e \u00e0 la m\u00e9decine. La ph\u00e9nom\u00e9nologie se pr\u00e9sente comme une m\u00e9thode (\u00ab&nbsp;descriptive&nbsp;\u00bb) visant \u00e0 suspendre les jugements pr\u00e9alables sur l\u2019exp\u00e9rience. Proposant de d\u00e9crire celle-ci pour acc\u00e9der \u00e0 sa pure manifestation, il s\u2019agit ensuite de d\u00e9crire les structures g\u00e9n\u00e9rales (essences) de l\u2019\u00ab&nbsp;appara\u00eetre&nbsp;\u00bb de la conscience<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#fn0005\"><sup>1<\/sup><\/a>&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0650\">[49]<\/a>. Ce mouvement a influenc\u00e9 des penseurs comme Martin Heidegger, Maurice Merleau-Ponty, Jean-Paul Sartre ou encore Alfred Sch\u00fctz, qui ont \u00e9largi son champ en int\u00e9grant respectivement des dimensions ontologiques, corporelles, existentielles et sociales.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">La pens\u00e9e de Husserl a eu un impact important sur la psychiatrie d\u00e8s les ann\u00e9es 1920. Karl Jaspers s\u2019est en effet inspir\u00e9 des premiers travaux de Husserl pour fonder une psychopathologie scientifique&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0660\">[51]<\/a>. Il cherchait \u00e0 d\u00e9crire l\u2019exp\u00e9rience du patient dans l\u2019ici et maintenant, et exposait la m\u00e9thode \u00ab&nbsp;compr\u00e9hensive&nbsp;\u00bb par laquelle le psychiatre pourrait \u00ab&nbsp;se mettre \u00e0 la place&nbsp;\u00bb de son patient pour se pr\u00e9sentifier son v\u00e9cu de la mani\u00e8re la plus rigoureuse. Plus tard, Eug\u00e8ne Minkowski et Ludwig Binswanger d\u00e9velopperont les premi\u00e8res descriptions ph\u00e9nom\u00e9nologiques des \u00e9tats psychotiques&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0450\">[9]<\/a>,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0705\">[60]<\/a>. Plut\u00f4t que de chercher des explications causales, les psychiatres ph\u00e9nom\u00e9nologues se sont ainsi attach\u00e9s \u00e0 comprendre comment les patients et les praticiens \u00e9prouvent leurs exp\u00e9riences.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"sect0115\">4.2.&nbsp;Objectifs en psychiatrie<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Les objectifs de la ph\u00e9nom\u00e9nologie psychiatrique sont de d\u00e9crire \u00ab&nbsp;ce que cela fait&nbsp;\u00bb de vivre telle ou telle exp\u00e9rience pathologique. Cette description m\u00e9thodique doit se tenir \u00e0 l\u2019exp\u00e9rience elle-m\u00eame, en tant qu\u2019elle est v\u00e9cue en premi\u00e8re personne et en t\u00e2chant de ne pas rabattre sur cette description des \u00e9nonc\u00e9s explicatifs ou normatifs.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">La ph\u00e9nom\u00e9nologie psychiatrique emploie la m\u00e9thode de l\u2019\u00e9poch\u00e8 pour \u00ab&nbsp;mettre entre parenth\u00e8ses&nbsp;\u00bb les th\u00e9ories et connaissances pr\u00e9alables de tel ph\u00e9nom\u00e8ne v\u00e9cu, afin d\u2019observer la mani\u00e8re dont les exp\u00e9riences v\u00e9cues se forment. Par exemple, en psychiatrie, certains champs comme les impacts sociaux des troubles mentaux ou les approches alternatives aux traitements biom\u00e9dicaux ont \u00e9t\u00e9 historiquement sous-financ\u00e9s et marginalis\u00e9s&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0685\">[56]<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Concernant l\u2019interconnexion entre \u00ab&nbsp;transformer&nbsp;\u00bb (psychiatrie critique) et \u00ab&nbsp;d\u00e9crire&nbsp;\u00bb (ph\u00e9nom\u00e9nologie psychiatrique), un exemple est celui de l\u2019exp\u00e9rience du chez-soi, promulgu\u00e9e par un mod\u00e8le comme \u00ab&nbsp;un chez-soi d\u2019abord&nbsp;\u00bb (<em>housing first<\/em>)&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0435\">[6]<\/a>,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0525\">[24]<\/a>. Ce mod\u00e8le postule que les conditions sociales et institutionnelles d\u00e9terminent et facilitent un acc\u00e8s au logement, et l\u2019habitat lui-m\u00eame, en retour, fa\u00e7onne l\u2019exp\u00e9rience de soi et du monde en influen\u00e7ant ainsi les formes de marginalisation.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Enfin, concernant l\u2019interconnexion entre \u00ab&nbsp;d\u00e9crire&nbsp;\u00bb (ph\u00e9nom\u00e9nologie psychiatrique) et \u00ab&nbsp;interpr\u00e9ter&nbsp;\u00bb (herm\u00e9neutique psychiatrique), un exemple est celui du processus de r\u00e9tablissement. L\u2019exp\u00e9rience v\u00e9cue prend forme \u00e0 travers un r\u00e9cit de soi, qui permet de redonner du sens aux trajectoires individuelles&nbsp;; l\u2019histoire personnelle se transforme en effet en un levier de r\u00e9tablissement et r\u00e9habilitation, reliant l\u2019exp\u00e9rience imm\u00e9diate aux significations construites au fil du temps&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0455\">[10]<\/a>,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0680\">[55]<\/a>. Ce croisement entre v\u00e9cu et narration trouve \u00e9galement un \u00e9cho dans la Daseinsanalyse de Binswanger, articulant ph\u00e9nom\u00e9nologie et compr\u00e9hension du sens existentiel de la souffrance&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0445\">[8]<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ces exemples, illustrant les interconnections prises deux \u00e0 deux entre les traditions, ne sont qu\u2019un aper\u00e7u des nombreuses interactions possibles entre les quatre mouvements propos\u00e9s pour cartographier la philosophie de la psychiatrie. Plut\u00f4t que de les opposer, il convient de reconna\u00eetre leurs interd\u00e9pendances, comme illustr\u00e9es dans la&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#fig0005\">Fig. 1<\/a>, qui sont le reflet de la complexit\u00e9 des pratiques psychiatriques, des connaissances qu\u2019elle mobilise et des probl\u00e9matiques qu\u2019elle rencontre, et de la n\u00e9cessaire h\u00e9t\u00e9rog\u00e9n\u00e9it\u00e9 des traditions qu\u2019implique la mobilisation de la philosophie de la psychiatrie au regard de la psychiatrie clinique.<\/p>\n\n\n\n\n\n<p class=\"wp-block-paragraph\" id=\"spar0055\">Fig. 1.&nbsp;La boussole des philosophies de la psychiatrie, une quadrature de la clinique psychiatrique entre explication, description, interpr\u00e9tation et transformation. L\u2019\u00e9pist\u00e9mologie psychiatrique cherche \u00e0 expliquer les ph\u00e9nom\u00e8nes psychiatriques, la ph\u00e9nom\u00e9nologie \u00e0 d\u00e9crire l\u2019exp\u00e9rience v\u00e9cue, l\u2019herm\u00e9neutique \u00e0 interpr\u00e9ter les r\u00e9cits et significations, tandis que la psychiatrie critique vise \u00e0 transformer les structures et pratiques en interrogeant les dynamiques de pouvoir. Nous proposons ici une posture avant tout didactique, conscients qu\u2019une telle quadrature ne constitue qu\u2019une boussole d\u2019orientation dans le vaste panorama et mani\u00e8re d\u2019agencer les diff\u00e9rents mouvements de la philosophie de la psychiatrie.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"sect0180\">8.&nbsp;Conclusions<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Proposer une cartographie didactique de la philosophie de la psychiatrie semble n\u00e9cessaire pour promouvoir la p\u00e9dagogie \u00e0 destination des \u00e9tudiants et praticiens en psychiatrie. Proposer d\u2019organiser ses d\u00e9bats et m\u00e9thodes autour de quatre mouvements de ce type soutient les enseignements et la transmission promus dans des formations comme le Dipl\u00f4me Inter-Universitaire \u00ab&nbsp;Philosophies de la psychiatrie&nbsp;\u00bb, cr\u00e9\u00e9 en France en 2019. Un tel programme ne se limite pas \u00e0 transmettre des savoirs conceptuels, mais int\u00e8gre \u00e9galement des comp\u00e9tences exp\u00e9rientielles et r\u00e9flexives&nbsp;\u2013&nbsp;invitant les participants \u00e0 interroger leur r\u00f4le et les dynamiques (inter)personnelles et institutionnelles&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0505\">[20]<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cependant, face \u00e0 un tel enchev\u00eatrement de questions et d\u00e9fis, le d\u00e9veloppement d\u2019une \u00ab&nbsp;clinique de l\u2019incertitude&nbsp;\u00bb appara\u00eet n\u00e9cessaire pour soutenir une approche humble et nuanc\u00e9e de la psychiatrie&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0425\">[4]<\/a>,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0470\">[13]<\/a>,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0485\">[16]<\/a>,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0635\">[46]<\/a>,&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0750\">[69]<\/a>. Cela implique d\u2019accepter trois sources d\u2019incertitude&nbsp;: l\u2019incertitude provenant des limites inh\u00e9rentes \u00e0 la connaissance m\u00e9dicale, celle provenant d\u2019une ma\u00eetrise incompl\u00e8te de cette connaissance, et celle provenant de la difficult\u00e9 \u00e0 distinguer ces deux niveaux, par le clinicien lui-m\u00eame. Le d\u00e9veloppement d\u2019une telle clinique de l\u2019incertitude pourrait reposer sur trois piliers&nbsp;: (i) apprendre et s\u2019entra\u00eener \u00e0 nommer ce que l\u2019on ignore, (ii) adopter une posture de curiosit\u00e9, favoris\u00e9e par l\u2019absence de compl\u00e9tude du savoir, (iii) coconstruire avec le patient une compr\u00e9hension mutuelle et d\u00e9pendante des points de vue divergents&nbsp;<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0625\">[44]<\/a>&nbsp;\u2013&nbsp;plut\u00f4t que de viser une v\u00e9rit\u00e9 absolue et exclusive. Une clinique de l\u2019incertitude en psychiatrie n\u00e9cessiterait alors d\u2019accepter la variabilit\u00e9 intra- et inter-individuelle en valorisant le doute comme outil central de communication.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Adopter cette posture reviendrait \u00e0 cultiver une \u00ab\u00a0clinique de l\u2019humilit\u00e9\u00a0\u00bb\u00a0<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003448725001611#bib0770\">[73]<\/a>, pouvant s\u2019exprimer par la reconnaissance des limites du savoir, l\u2019int\u00e9gration des \u00e9motions li\u00e9es au doute, l\u2019attention aux dynamiques relationnelles avec les patients et une volont\u00e9 d\u2019apprentissage constant.
